Losing one of our beloved Cesky Fouseks, whether to accident, disease or old age is a heartbreaking experience. We appreciate your taking the time to provide information about your dog’s passing. The data you provide is valuable to our breeding committee for monitoring the health of the breed.
This survey concerns factors associated with the dog’s death and quality of end-of-life. The survey will request things like your dog’s registered name, whelp date, and registration number. Accuracy will be aided by having veterinary information handy. Please access your dog information before proceeding.
